EverGlow Dental – Terms & Conditions

By booking an appointment with EverGlow Dental, either directly or via the online booking portal (the “Portal”), you agree to the following terms and conditions:

1. Patient Responsibility
You are responsible for providing accurate and up-to-date personal and medical information during booking and consultation. If someone else is completing forms on your behalf, you confirm they are authorised to do so.

2. Use of the Portal
Our online booking system (Dentally Portal) is provided by Henry Schein One. Information entered into the Portal is transferred to EverGlow Dental to manage your care. The Portal provider does not retain or use your personal information for marketing or unrelated purposes.

3. Privacy & Data Handling
For details on how we collect, store, use and share your information, please refer to our Privacy Policy.

4. Access to Your Information
You may request access to, correction of, or deletion of your personal data at any time by contacting EverGlow Dental at info@everglowdental.com.au. We will respond within a reasonable timeframe and in accordance with Australian privacy law.

5. Cancellation Policy
Appointments cancelled or rescheduled with less than 24 hours’ notice may incur a cancellation fee. Please see our Cancellation Policy for details.

6. First Point of Contact
For any questions or concerns regarding your personal information or your care, EverGlow Dental is your first point of contact, not the Portal provider.

These terms are reviewed regularly and may be updated.
